Travel Forms
Permission forms may be generated for any trip by clicking on the event title on the troop calendar. Alternatively, you may use the generic permission form below. The troop may not transport a Scout without a permission form signed by a parent or guardian.

Medical Forms

The troop must have an up to date Class 3 medical form on file for all Scouts attending outings. This form is good for one year. It does not require a full physical exam, an office visit checkup is often all that is required and this can have taken place within the preceding 12 months.
